Family Health Centers

Family Health Centers is a nonprofit community health center with 8 locations throughout Louisville Metro. FHC provides primary and preventive care to adults, women and children. Our vision of service is to provide you and your family with the same care and attention we would want for our families and ourselves. We are open to anyone who needs health care. Our sliding fee discounts are for individuals or families below 200% of the Federal Poverty Guidelines. Our minimum fee for individuals at 100% FPL or below is $20.00 for medical services and $30.00 for dental services.
